Arkansas has a limited list of qualifying conditions for which your medical doctor can prescribe medical marijuana. If you are unaware of which conditions qualify, they are as follows:
- Cancer
- Glaucoma
- Positive status for human immunodeficiency virus/ acquired immune deficiency syndrome
- Hepatitis C
- Amyotrophic lateral sclerosis
- Tourette’s syndrome
- Crohn’s disease
- Ulcerative colitis
- Post-traumatic stress disorder
- Severe arthritis
- Fibromyalgia
- Alzheimer’s disease
- Cachexia or wasting syndrome
- Peripheral neuropathy
- Intractable pain which is pain that has not responded to ordinary medications, treatment or surgical measures for more than six (6) months
- Severe nausea
- Seizures including without limitation those characteristic of epilepsy
- Severe and persistent muscle spasms including without limitation those characteristic of multiple sclerosis
- and any other medical condition or its treatment approved by the Department of Health
These have been taken directly from healthy.arkansas.gov website so they are accurate as of this writing. But having one of these conditions does not automatically qualify you to use medical marijuana (MMJ).
In order to receive a card allowing you to purchase MMJ you must include the following with your application:
- The application form.
- The completed Physician Written Certification.
- Photocopy of your Arkansas issued driver’s license or state ID (remember: name and address must match what is listed on your Arkansas driver’s license or ID).
- The nonrefundable $50 application fee.
The application can be found on this page and is entitled “Patient Application”. It is a downloadable PDF file. The only health care providers that can certify you for this medicine are medical and osteopathic doctors, and they are not required to do so. You must have an actual doctor / patient relationship and some are more willing than others to help you acquire MMJ.
Once you receive your medical certification you have 30 days to submit the above list to the Arkansas Department of Health before it expires. If you do not submit it on time you will be forced to get a new certification before receiving a card for MMJ.
Online applications are preferred and that link is here.
